DriveWell Go Program by Cambridge Mobile Telematics

1. I confirm I have read and agree to the provisions of the Mobile App License Agreement.
2. I confirm I have read and agree to the provisions of the Privacy Policy.
3. I explicitly consent to the collection, storage and processing of data by Cambridge Mobile Telematics Inc. about my driving habits when using the App and Services, which includes profiling and the collection of aggregate statistics on my driving habits and use of the Services.
4. I explicitly consent to Cambridge Mobile Telematics Inc. sharing data about my driving habits when using the App and Services, and aggregate statistics collected on my driving habits and use of the Services with its partners who wish to send me electronic communications about the telematics program that I have registered to participate in.

You can withdraw your consent to the collection of driving habit information and/or receiving electronic marketing communications from our partners at any time by sending an email to privacy@cmtelematics.com.

I wish to enter into a legally binding agreement to use the Software subject to the Mobile App License Agreement and the Privacy Policy.
